Personal Information Collection (PIC) Statement
 
  1. The personal data collected in this application form will only be used by Dah Sing Bank Limited to process your application and will be kept for 6 months.
  2. Please make sure that the data are complete and accurate. If you fail to provide us with the data requested or if the data provided is inaccurate or incomplete, we may not be able to process your application.
  3. We use encryption in providing secure transmission of personal data on the Internet. In avoidance of risk in transmission, you may complete the download application form and return to any Dah Sing Bank Branch or mail to Dah Sing Bank Ltd., GPO Box 333, Hong Kong or fax to 2828 8060.
  4. Information we collect about you may be disclosed to other companies under certain circumstances (please refer to the ¡§Terms & Conditions¡¨).
  5. Subject to exemptions under the Personal Data (Privacy) Ordinance, you have the right to contact the Data Protection Officer of the Bank to gain access to and request correct to your personal data. If you wish to do so, please contact our Data Protection Officer, 34/F Dah Sing Financial Centre, 108 Gloucester Road, Wanchai, Hong Kong.
