Please note that your above choice applies to the direct marketing of the classes of products, services and/or subjects as set out in paragraph G of the Bank's Notice to Customers relating to Customers' Data ("Notice") made available by the Bank to you from time to time. Please also refer to the Notice on the kinds of personal data which may be used in direct marketing and the classes of persons to which your personal data may be provided for them to use in direct marketing.
 
Remarks:
  1. Please note that this application is only for existing VIP / i-Account "magnetic stripe" ATM cardholder.
  2. Upon successful application, you will receive Dah Sing Octopus VIP / i-Account App Card within 8 working days after card-application. Once the Dah Sing Octopus VIP / i-Account App Card is received, you are advised to:
    2.1.  activate the Dah Sing Octopus VIP / i-Account App Card (details will be enclosed in times of card issuance), the AAVS Activation Letter will be mailed to you after you have activated your Dah Sing Octopus VIP / i-Account App Card;
    2.2.  cut your old magnetic stripe ATM card into two halves across its magnetic stripe; and
    2.3.  bring along your Dah Sing Octopus VIP / i-Account App Card, identification document and original copy of AAVS Activation Letter to activate the AAVS at any Customer Service Centre of MTR station.
    2.4 Please note that the Bank will terminate your existing magnetic stripe ATM card and the Dah Sing Octopus VIP / i-Account App Card if you do not activate the new Dah Sing Octopus VIP / i-Account App Card within 4 months after card issuance.
  3. You must have a valid mobile phone record with Dah Sing Bank ("the Bank") to receive notices. If you have changed or never provided your mobile phone number to the Bank, please visit any Dah Sing Bank branch to update information.
  4. The Bank might send alert message to your mobile phone via "Short Message Service" (SMS) relating to the Dah Sing Octopus VIP / i-Account App Card, if necessary. The Bank will not be responsible for any SMS "recipient charges" charged by mobile phone service providers, regardless it is for local or overseas SMS.
  5. You must hold a valid VIP i-Account / i-Account and a valid "magnetic stripe" ATM card in times of card issuance.
  6. The Bank and / or Octopus Cards Limited ("OCL") might not accept your application if the Bank and / or OCL find that you have an overdraft or any data mismatch with the latest record of the Bank and / or OCL in times of card issuance.
  7. The e-mail address provided above will not affect the e-mail address(es) that you maintain with Dah Sing e-Banking Service for statement alert and other system notices.
  8. The Bank reserves the right to amend the above terms and conditions thereof at any time without prior notice.
  9. In case of any disputes, the decision of the Bank shall be final and conclusive.
  10. In case of any discrepancy between the English and Chinese versions of the terms and conditions, the English version of the terms and conditions shall prevail.

Declaration and Agreement for Dah Sing Octopus VIP / i-Account App Card ("Dah Sing Octopus App Card"):
I declare that by applying for the Dah Sing Octopus App Card and activating Automatic Add Value Service, I have read, accept and agree to, and shall be bound by, Terms and Conditions applicable to holders of the Dah Sing Octopus App Card specified and amended by the Bank from time to time ("Dah Sing Octopus App Card Terms and Conditions"), the Conditions of Issue of Octopus and Octopus Automatic Add Value Agreement issued by Octopus Cards Limited ("OCL") (collectively, as amended from time to time, the "Agreements"), copies of which are attached in the above embedded hyperlinks and the Agreements provided by OCL can also be obtained from OCL website at www.octopus.com.hk. I hereby authorize the Bank to disclose my personal data or information to OCL as well as other parties stated in "Personal Information Collection Statement" in the Octopus Automatic Add Value Agreement for the purposes stated therein.
