申請領取月結單客戶需繳付手續費予本行。此申請將於所有有關款項由指定戶口扣除後方為生效。
倘若發現以上申請所填報之資料有錯誤，本行保留權利向閣下索償或由閣下之戶口扣除所有有關損失之全數。
This application is effected only after all fund related is debited from the instructed account successfully. The applicant undertakes to reimburse the Bank any charges owing to the request of statement as mentioned above.
In case of error or irregularity being found regarding this application, the Bank reserves the right to debit your account or claim on you at once the whole amount of any cost/loss caused by it.
